Kidney Disease – The Role of the GP in transition to end stage care

This seminar looks at the issues around progression to CKD stages 4 to
5 – dialysis dependency, pre-emptive transplantation. How do you reduce the morbidity and mortality in this transition? When do you refer? How do you assist the patient to decide on peritoneal or haemodialysis? When do you create vascular access? How do you deal with someone medically suitable for renal replacement who decides not to proceed with therapy? The role of the GP in all these areas is increasingly important and influential.
