STROKE MANAGEMENT - BEST CARE GUIDE

This Active Learning Module (ALM) will cover three topics:

  • TIA and early assessment
  • Antiplatelet therapy for secondary stroke prevention
  • Atrial Fibrillation – Preventing fatal and disabling stroke in patients with AF

There will be an update on any relevant new and emerging evidence for example the use of the new anti-thrombin inhibitors in the treatment of NVAF.

Each topic uses a case based approach to discuss current clinical evidence and guideline recommendations. The ALM includes discussion on early assessment, risk stratification, use of risk stratification tools, secondary prevention, anticoagulants and antiplatelet therapy and management of patients. The ALM highlights the GP’s role in providing continuity of care in complex patients with multiple morbidities.

Each topic runs for 1½ hours

Please note that this ALM can be completed in 1 day. To qualify for this ALM (category 1 – 40 points) participants must attend all 3 topics as well as complete the post conference activity.

Each participant will be provided with five PENCAT recipe cards for your practice. These PENCAT recipe cards are meant to be used for appropriate prescribing of antiplatelet, cholesterol lowering and antihypertensive therapies, using a GP management plan for stroke patients, and referring stroke patients to join peer support groups.

1. TIA and early assessment

The module is case based and presents information on current clinical evidence and guideline recommendations including:

  • The risk of stroke following TIA
  • The signs and symptoms of TIA
  • The risk of recurrent stroke after TIA
  • The use of the ABCD2 tool to stratify risk of stroke
  • Transfer to an appropriate healthcare facility
  • Appropriate secondary prevention strategies in patients with TIA
  • Barriers to optimal stroke care

2. Antiplatelet therapy for secondary stroke prevention

The module is case based and presents information on current clinical evidence and guideline recommendations including:

  • Expert consultation
  • The use of antiplatelet agents in stroke patients
  • Data from landmark clinical trails on commonly prescribed antiplatelet agents
  • Detailed discussion on the different antiplatelet agents
  • The differences and similarities between antiplatelet agents
  • The GP’s role in long-term Mx of stroke survivors
  • Managing other risk factors and co-morbidities

3. Preventing fatal and disabling stroke in patients with atrial fibrillation

The module presents information on:

  • Stroke risk and outcomes with non-valvular atrial fibrillation (NVAF)
  • CHADS2 score and risk of stroke
  • Evidence-based use and choice of antithrombotic treatments including the newer treatments in NVAF
  • Common barriers to stroke risk reduction in NVAF
  • Strategies to overcome barriers in the management of NVAF
  • Quality control of anticoagulation in atrial fibrillation
  • Management on warfarin, INR and bleeding risk
  • GP survey and BAFTA study
